Step-by-Step Process

Follow these simple steps to access Support at Home funding and begin receiving the care and services you need.
Contact Maison Care Services
Call Maison Care Services on 0406 084 141 (free call) to register and arrange your aged care needs assessment. This is the starting point for accessing any government-funded aged care service. How you can contact My Aged Care:
  • Phone: 0406 084 141 — Monday to Friday 8am to 8pm, Saturday 10am to 2pm (local time)
  • Online: www.myagedcare.gov.au
  • In person: Book a face-to-face appointment with an Aged Care Specialist Officer (ACSO) at a Services Australia centre — call 0406 084 141
  • Via a Care Finder or Elder Care Support Worker — they can help you navigate the system

Screening
Our Affiliate Care Services Australia will have a brief conversation with you about your needs and situation. If you appear eligible, you will be referred to an assessment organisation for a full aged care needs assessment.

Aged Care Needs Assessment

A trained assessor will contact you and arrange to meet — in person at your home, by phone, or via video. This assessment is free and helps identify what support you need.

During the assessment:

  • You will be asked about your daily living activities, health, and goals
  • The assessor uses the Integrated Assessment Tool (IAT) to understand your needs
  • You will be involved in developing your Support Plan
  • You can have a family member, carer, or support person present

The assessment results in a Support Plan that recommends the type and level of services appropriate for you.

Approval and Classification

Based on your assessment, an assessment delegate makes a decision on:

  • Whether you are eligible for government-funded aged care services
  • Which service group(s) are appropriate (e.g. home support, assistive technology, home modifications)
  • Your classification level (1–8 for ongoing Support at Home)
  • Your priority category (Urgent, High, Medium, or Standard)

You will receive a Notice of Decision letter. Your priority category determines how long you may wait before funding is allocated.

Funding Allocation

Once your funding is allocated, you will receive a letter from My Aged Care confirming your Support at Home budget.

⏱️ IMPORTANT TIMELINE:  You have 56 calendar days from the date your funding is allocated to choose a provider and start services. If you need more time, contact My Aged Care to request a 28-day extension (84 days total). If services do not start within this window, your funding is withdrawn and you will need to re-enter the system.

Choose Maison Care Services as Your Provider
Under Support at Home, you work with a single registered provider. Choose Maison Care Services and we will:
  • Assign you a dedicated care partner
  • Contact you to arrange your first meeting
  • Develop your care plan and budget together with you
  • Coordinate and begin delivering your approved services
